About General Services Division
The ADOA Badging Office provides new badging services that include new badges, changes to badge access, replacement badges and cancellation of badges. The Badging Office is in the lobby of 100 North 15th Avenue, open Monday through Friday, 8:30 a.m. to 12:30 p.m.

New or replacement badge
To get a new or replacement badge, you will need (1) a badge request form signed by your agency building coordinator or other authorized signator that identifies the access rights and (2) a legal form of identification such as a driver’s license, military ID, or passport. This is the same process for changing badge access rights.

Care of state badge
Treat your badge as a credit card – do not leave it in extreme heat or direct sunlight, and do not expose it to magnetic fields.

Cancel a badge
Report to your supervisor so that they can notify the badging office and get it cancelled either by email or submitting the badge request form indicating to cancel.

Lost or stolen Badges
Report the missing badge to your supervisor so that they can notify the badging office and get it cancelled, and then go to your badging coordinator so that they can issue you a badge request form. Bring that signed form to the badging office, and another badge will be issued.

Report a problem
If you are experiencing difficulties with your badge, report the issues to your supervisor so that they can notify the badging office.

Badge expiration date
A badge expires four years after the date of issue.

Badge readers not working
Call the help desk at 602-542-4594 to request a work order.
The FOAM Maintenance Unit repairs and maintains the existing infrastructure and equipment of three million square feet of buildings, for example, sinks, faucets, toilets, lights, windows, and doors.

Power outages and emergency repairs
Provide your building coordinator with the location and detailed description of the problem. You also may contact the Customer Support Desk at (602) 542-4594. For after-hour emergencies, contact Capitol Police at (602) 542-4580.

Electrical maintenance and repair
Maintenance provides repair or replacement of existing electrical receptacles, switches and other motors or devices.

Plumbing
Maintenance provides repair or replacement of existing infrastructure and equipment, for example, sinks, faucets, toilets, piping, motors and pumps.

Painting and decorating
Maintenance provides repair or replacement of existing dry wall, plaster and paint as needed. Maintenance will paint in our select standard colors only. Place a work order to hang any pictures.

Elevators
FOAM contracts with an elevator company to provide elevator maintenance, repair and testing. For an elevator entrapment, use the emergency elevator phone inside the elevator to contact Capitol Police. Do not attempt to exit until qualified personnel instruct you. For an elevator repair, provide your building coordinator with the location and detailed description of the problem. You also may contact the Customer Support Desk at (602) 542-4594. For after-hour emergencies, contact Capitol Police at (602) 542-4580.
All conference room reservations are restricted to use by state agencies only. There is no fee for a state agency to reserve a conference room. ADOA manages the following conference room in the Capitol Mall:

1700 W. Washington: lobby, first floor, second floor, fourth floor and basement (Grand Canyon Room)

100 N. 15th Avenue: ADOA common area conference rooms
The FOAM Custodial Unit provides janitorial services for break rooms, cubicles, offices, lobbies, and restrooms. FOAM hires inmate labor as a cost savings to the State and the inmates are supervised at all times. FOAM closely coordinates with the Arizona Department of Corrections, which carefully screens the inmates.

Emergency clean-up
An emergency clean-up is defined as a water leak or flood, bodily fluids, or any chemical that may cause a permanent stain.

Cleaning schedule
As of February 2009, the FOAM Custodial Unit is providing a reduced level of custodial services as a cost savings to the State and provides options for additional services as needed.
Restrooms spot clean and restock daily; fully clean twice a week Trash empty at each desk twice a week; empty break room trash daily Recycle empty at each desk once a week Dusting twice a month Vacuuming twice a month Sweeping twice a month Mopping twice a month Carpets spot clean once a month as needed

Perishable items
All employees should discard perishable items in the break room or lunchroom trashcans. This is the only area with daily trash service.

Carpet cleaning and stripping/waxing of floors
At this time, FOAM is not waxing and stripping floors and we are not cleaning carpets. We will monitor for spots and stains, which the crew will clean once a month. Place a work order for any spills that may cause permanent damage. Agencies may elect to pay for their own services through a tenant-improvement transfer request. We will review additional carpet and floor service requests on a case-by-case basis.

Dusting
Custodial workers will not touch or remove anything on a desk. To ensure that the custodial dusts your area properly, clear away any papers and move any objects from the area that you would like to have dusted. Dusting is schedule for twice a month.
The Equipment Repair and Tool Center provides valuable services to the state. They are in charge of repairing broken equipment, including carts, and loaning tools to employees.

The time a cart will remain out of service depends on the nature of the problem and whether any parts need to be ordered. The wait for a cart to be picked up by our department for mechanical issues can vary depending on the availability of personnel.

Tool loans
Tool loans are issued directly to the employee, and can be issued indefinitely.
The FOAM Grounds Unit maintains approximately three acres of land on the Capitol Mall and on certain satellite properties. The services provided include landscaping and water conservation.

Inmate Labor
Tool loans are issued directly to the employee, and can be issued indefinitely.
The FOAM HVAC Unit repairs and maintains the existing HVAC equipment, including routine services, service requests, temperature adjustments, and fire alarms.

Place an HVAC request
To place a work order for the services listed here, provide your building coordinator with your location and contact information, and describe request in detail. You also may contact the Customer Support Desk at (602) 542-4594. For after-hour emergencies, contact Capitol Police at (602) 542-4580. Work is completed within 24 hours unless parts must be ordered.

Equipment maintenance, repair and warranties
HVAC will call a contractor and complete the repairs within 48 hours of receiving a work order.

Equipment Replacement
An HVAC mechanic identifies the problems and gives a recommendation on equipment replacement. HVAC usually will contact a contractor to provide equipment replacement. The timeframe will vary depending upon the funding availability and severity of the problem.

Office Temperature adjustment
HVAC will perform temperature adjustments within 24 hours of receiving a work order, unless parts must be ordered.
The FOAM Physical Security Unit provides services duplicate keys for furniture locks and door locks. FOAM Physical Security maintains building security only; refer to ADOA Information Services Division for computer and server security.

Keys (new or replacement)
Door Keys: To request a duplicate door key, a manager can call the Customer Services desk at 602-542-4594 and place a work order. Once cut, the requesting manager may issues the key as needed. An Assistant Director must place the call in order to receive a master or grand-master key.
Furniture Keys: First contact the agency building coordinator, as the key may be covered by a contract. Otherwise, call the Customer Service Desk.

Secure or unlock buildings
Contact Capitol Police at 602-542-4580.
The FOAM Special Events Unit is responsible for approving permits for solicitations and special events, as well as reserving multiple conference rooms in the Executive Tower (see section on Conference Room Reservations). The Special Events Office will receive and process the application, and will issue a permit when all required information has been received and approved. As of February 2009, the FOAM Special Events Unit is providing a reduced level of services as a cost savings to the State. Special Events has eliminated the services for room set-ups and no longer supplies microphones, table linens and skirts.

Is there a Fee?
There is a $200 fee per section for non-state agency use of Wesley Bolin Plaza, and a $2,800 fee to use the entire plaza. Capitol Police will charge for additional security when required for large or after-hours events.
The warehouse and supplies department makes the purchases of equipment, parts, and supplies. You must place an order with an MTD in order to purchased items.

Place an Order
A completed Material Transaction Document (MTD) must be filled out with the correct information to place an order. Orders are placed either with a blanket purchase order or a purchase card. Once order is placed depending on price and availability, your parts will be received within an hour, or up to 3 weeks. -
Cost of Supplies
Depending on which vendor the cost of the item can vary. It is best to have a Facility Support Center Staff get the best price for you.
